Echocardiography in Hypertrophic Cardiomyopathy

Introduction

  •  

Goals of Echo

  1. Determine presence, extent and pattern of left ventricular hypertrophy
    1. 15mm or greater diagnostic (13-14mm if family history)
  2. Image mitral valve and apparatus
    1. Primary valve abnormalities (longer leaflets)
    2. Apically displaced and hypertrophied papillary muscles
    3. Systolic anterior motion of mitral valve
    4. Mitral regurgitation from SAM or primary mitral valve abnormalities
  3. Confirm LVOT obstruction
    1. CW at rest and with provocation (valsalva most common)
  4. Exclude
    1. HTN, AS and infiltrative/storage diseases
